California Healthy Kids Survey
The survey gathers information on behaviors such as physical activity and nutritional habits; alcohol, tobacco, and other drug use; school safety; and environmental and individual strengths and assets. You may examine the 65-item Elementary School Questionnaire in the school office or online at www.dusd.net/chks
